Immunization Clinic

We are excited that Giant Eagle Pharmacy will be hosting our Immunization Clinic. For the clinic to run as efficiently as possible, please see the following information below:

• Print and complete an Administration Form prior to clinic (forms are available at the front desk of the library).  Bring the form with you to the clinic to help speed up the process. If you are getting both the flu and the covid vaccine, we will need a completed Flu and COVID administration form. However, if you are unable to do so, forms will be available at the clinic.  
• Bring all of your insurance cards with you to ensure proper billing.
• Bring your Giant Eagle Advantage Card in order to receive the $5 grocery shopping trip reward (Giant Eagle phone app will suffice) 
• We will have all flu shot types- Trivalent flu shots, 65+ flu shot, and the New covid Booster
